Application Instructions

  1. Fill out the “Application and Self-Certification Form” [Download form here].
  2. Sign and date the certification (found above).
  3. Return to this webpage to submit the application.
  4. Within the application submission section of the page, fill in your answers to the application background questions.
  5. While on the webpage, upload: 
    1. PDF of the signed Application and Self-certification (found above). This includes Direct Deposit/ACH information to allow funds to be transferred to your bank account
    2. PDF of Cancelled Check to allow for Direct Deposit/ACH Payment 
    3. The W-9 form for your agency. Download a copy of a blank W-9.
    4. PDF of 501 (c) (3) current certification or other proof of current status, e.g. IRS Form 990 or 990-N
  6. Make sure to hit the “Start Upload” button to complete the upload of your files. 
  7. Once uploaded, ensure you click the “Submit” button to finish the application process. 
  8. A confirmation email will be automatically sent to the email provided.
  9. Please anticipate 4 to 6 weeks for applications to be processed.
